TERMS AND CONDITIONS 1. Rules: The child and both Parent/Guardian agree to abide by all the rules and regulations set forth to participate in program including, but not limited to those relating to enrollment and withdrawal of child. 2. Behavior Management: This program has a NO Tolerance Policy for verbal and/or physical abuse as well as any forms of violence. In addition, Key of David Worship Arts Camp is a BULLY-FREE ZONE! Any kind of bullying is unacceptable at Key of David Worship Arts Camp. If issues arise, we immediately speak to the individuals involved and try to resolve it (parents will be notified). If it persists, campers are removed from camp activities to reinforce the seriousness of learning to resolve issues without conflict and parents are notified that this step was taken. If the 2nd resolve is unsuccessful and a 3rd conflict occurs, the individual(s) will be terminated from the program for the duration of the camp and no money refunds will be issued. Key of David Worship Camp reserves the right to dismiss, in its sole discretion, any child whose condition, conduct, influence and/or behavior is deemed inappropriate or detrimental to the best interests of the program and/or its attendees. Those who violate camp rules and guidelines will not receive refunds. 3. Emergencies: Medical care will be provided by individual(s) who are trained and certified in CPR, First Aid and Blood Borne Pathogens. Parent/Guardian grants permission to utilize medical treatment. Should it be necessary for the well being of the student(s) to use outside medical care, the parent/guardian will pay all expenses involved. In addition, the parent/guardian will pay any dental, orthodontic, or optical work needed. 4. Sign-In Arrivals/ Sign-Out Dismissals: Every child must be signed in daily for camp and signed out daily upon dismissal by the individual(s) who are listed on the registration form, with prior written permission granted to release child. NO EXCEPTIONS. 5. Permission Granted to Participate: Parent/Guardian grants student(s) permission to participate in all camp activities and classes and understands that accidents and/or injuries may occur in the natural course of participation in such activities. Permission is also granted to the camp to use in promoting the program and in other ventures directly relating to the camp, digital photographic, video and audio images or likenesses of student(s); and statements, articles, names, music, art, photograph, audio recordings and videos created by student(s) or originating from any program related activity. 6. Personal Articles: The camp is not responsible for personal belongings or equipment while in or participating in any of our program activities. I have read and accept all of the terms and conditions set forth in the Registration Form. I also understand that in consideration of permission for my child(ren) to participate in the camp, acknowledge that I, my spouse, my personal representative, guardians, do hereby, waive, and discharge the Key of David Worship Arts Camp and Consuming Fire Christian Center including its volunteers and officers from all claims, actions and liability from all and any claims resulting from the inherent risks of camp activities and for any personal injuries known or unknown, caused by or arising out of the involvement of my child(ren) activities. *

PAYMENT AND CANCELLATION POLICY All registration fees are non-refundable. Registration fees are required to be paid in full at the time of enrollment to secure placement. Requests for payment plans must be sent to the Key of David Worship Arts Camp Director at info@keyofdavidcamp.com. If a payment plan is in place, all balances must be paid in full by the first day of camp. Payments are accepted in cash, check or by PayPal. (Please note: PayPal payments will have a processing fee. In addition, all returned checks will incur fees to be paid by the parent/guardian.) Make checks payable to Key of David Worship Ministry. *

I hereby authorize Key of David Worship Arts Camp to publish the photographs taken of me and/or the undersigned minor children, and our names, for use in print, digital and other forms of media. I release Key of David Worship Arts Camp from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize Key of David Worship Arts Camp to use their photographs and names. I acknowledge that since participation in publications and websites produced by Key of David of Worship Arts Camp is voluntary, neither the minor children nor I will receive financial compensation. I further agree that participation in any publication and website produced by Key of David Worship Arts Camp confers no rights of ownership whatsoever. I release Key of David Worship Arts Camp, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.
